This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Important: Please do not include sensitive medical or personal health information in any web form on this site (Contact, Schedule a Tour, Care Quiz). If you need to share protected health information, please call us directly at (318) 281-6933.

Our commitment

Cherry Ridge Skilled Nursing Facility is required by law to maintain the privacy of your protected health information (PHI), provide you with this notice of our legal duties and privacy practices, notify you following a breach of unsecured PHI, and follow the terms of the notice currently in effect.

How we may use and disclose your PHI

  • Treatment — to provide, coordinate, and manage your care.
  • Payment — to bill and collect payment for services provided.
  • Healthcare operations — quality assessment, staff training, accreditation, and similar activities.
  • As required by law — public health activities, abuse or neglect reporting, health oversight, judicial proceedings, law enforcement, coroners, organ donation, research with appropriate safeguards, to avert serious threats to health or safety, military and veterans, workers' compensation, and similar legally permitted disclosures.
  • To family and friends involved in your care — unless you object.

Other uses and disclosures — including most uses for marketing, sales of PHI, and most uses of psychotherapy notes — require your written authorization, which you may revoke at any time.

Your rights

  • Inspect and copy your PHI.
  • Request an amendment if you believe your PHI is incorrect or incomplete.
  • Request an accounting of certain disclosures.
  • Request restrictions on certain uses and disclosures.
  • Request confidential communications by alternative means or at alternative locations.
  • Receive a paper copy of this notice on request.
  • Be notified of a breach of your unsecured PHI.

Complaints

You may file a complaint with our Privacy Officer or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for PHI we already have as well as any PHI we receive in the future. The revised notice will be posted in our facility and on this website.
